2010 COSFC Entry Form
An honor system is used for entering fish into this tournament. This form can be used by all  member to  fill out, and copy and paste into an email  to the Web Master for posting below. 
However, members with no access to a computer can provide the Tournament Chairperson
(either mailed or handed to the TC or a Club Officer at the monthly meeting)
a weight ticket or a manually filled out version of this entry form with similar information so your catch. information can be entered.
Member entry categories are: Youth Anglers, Lady Anglers, and Regular Members
The 2010 Tournament began 11/06/09 ends 11/012/10.

    Certificates and or Plaques will be awarded at the Dec. Meeting.
